Get proper rest; Take regular breaks
Rest: Sleep deprivation snatches your ability to focus. Getting proper rest regularly is crucial for maintaining your concentration. 7-9 hours of daily sleep is recommended. Breaks: You already know that 'all work and no play' is not a great idea. So take regular breaks while studying or working. Go out for walk or do whatever that works for you. Napping could help too.
Avoid stress; Quit multitasking

Try relaxing activities like yoga, meditation, etc. Quit multitasking: This one is a no-brainer. When you open too many tabs in your brain, it suffers to co-ordinate well and thus the quality of your work takes a dip.
