To update your name or any other identity-related info on your Aadhaar card, you have to provide a proof of identity.
Valid documents can be your passport, voter ID card, PAN card or driving license.
These documents should have your photo and full name clearly printed on it, exactly as you want it to be updated on your Aadhaar card.
Address proof

If you want to update your address on Aadhaar card, make sure you have valid proof of address.
Documents like utility bills (electricity or water), bank statements with recent transactions, rental agreements, or property tax receipts are acceptable.
Just make sure they are not older than three months from the date of application.
Birth proof

In order to change your birth date in Aadhaar records, you need to furnish a valid proof of your DOB.
A birth certificate issued by the municipal authority or any office of the government of India is the most authentic proof.
If you don't have a birth certificate, an SSLC book/certificate or a passport (if they mention your correct DOB) can be used.
